I have to wonder about the expense of operating their own setup vs. shared space in a big medical building though.

I've been with the same dental practice since moving to Vancouver as a kid. The dentist I had growing up is now semi-retired, but his daughter became a dentist as well and they now share the practice. She took over my file a while back and I'm happy with her and get to keep the same hygienist, so it was a seamless transition. Also, since she is no more than a decade older than me, I expect that I will be a client of hers for many years to come and wouldn't be surprised in the slightest if her sons or daughters may eventually become dentists and join the practice as well. I'm helping fund a dental empire.
I've never had the impression that very much was shared between practices in a medical building. My doctor is in one and his is a self-contained practice. It must be convenient to some to have one's doctor, specialists, a lab, and a pharmacy all in the same building, but it is rare that I use any of those on a visit to my doctor, so their proximity is of little utility to me. The Neelu Bachra office building on Broadway between Cambie and Ash is set up to essentially be a medical-dental office building and the office suites are strata title and were demised in plan to accommodate standard sized medical-dental practices.
