There can be more than one level of blame for things. The bicyclist obviously did something stupid. And maybe the street designers also designed something unsafe. The latter didn't commit manslaughter, but bad street design certainly contributes.
As for hospital design, I agree that aesthetics aren't in the top handful of considerations. It's more about adjacencies and other factors that support care and efficiency (particularly with the recent Lean push), including factors like operational cost. Hospitals care about image, but their buildings don't include a lot of extra money for image.
