Very unfortunate. My deepest sympathy towards the workers families.

I don't know the full story, but I can't help but think the safety line was not being utilized by the worker. The reason I think this has much to do with observing the workers travel along the scaffolding at the King George. The workers almost always connect their safety lines, but I have noticed a few times where they weren't connected, one slip on a icy wood plank...and that's it.
