We've been a landlord several times. You don't actually need tenancy forms, but they are good for your own protection. Here is a link to standard forms by the Residential Tenancy Branch (BC Government)

The form is free. Strata fees are always paid by the owner, but usually Hydro/cable/phone is paid by the tenant. It should be specified in the tenancy contract. (check the boxes of on the form of what is included).
The only exception to hydro/cable/phone not being included, is that sometimes, if you're renting out to students, it is common that everything be included, so you can increase the rent amount, and include all the services.
Damage deposit is also specified on the form, and it cannot exceed half a months rent in BC.
