There's also issues with water and salt ingress causing issues poorly designed or sealed units.
Most of the issues cars were having was attributed to defects during the battery construction. The LG batteries in the Chevy Bolt and various Hyundai products for instance had problems with the well separators getting folded as the cells were built, making it so dendrites could bridge the anode and cathodes, then cause a short.
The issue occurred during charging because charging grows the dendrites.
