It represents situations where they cannot find out what the source was. Most of it is just people directly linking to something (like from their bookmarks or typing it in), or if they view it from an instant messenger (I know that links clicked in msn will show up under unknown source) or an email client. It's basically a catch all for things that aren't websites. It's simply unusual for a photo to have almost all of its hits from an unknown source, and for those hits to average 15 a day every day.
I'm not sure when stats was released but I think it was only a couple years ago. If you download the stats in a csv file, it only goes back to the summer of 2008.
