Yes, I recall reading that too. I meant why in terms of the oversight part of bridge engineering -- why didn't the contractor's engineers, or MTO's, discover the deficient parts before they were installed, or when inspected after installation.
I don't recall that ever being made public, and an internal investigation into that part of the build process would have happened.
