How do you measure a building with a crane on top?

When measuring a building, its entire height and whatever is on its roof are taken into account, but what happens when there is a crane on top of it to clean windows that changes height depending on its position?

I have seen many buildings that at their highest point have a crane that does not have a fixed height since it goes up or down.

No, whatever is on top of the roof is not taken into account in the "official" height. Only architectural/ structural features. A separate unofficial entry for the crane/ building maintenance unit height can be created though.
