Built on the highest elevation site in downtown Winston-Salem, at the top of a steep ridge. The architect selected for the building was the leading church architect in the United States. After seeing the site, architect Ralph Adams Cram said "A man does not have many chances in a lifetime to build a church on a location such as this."

The granite is from Plymouth, Massachusetts and the building is trimmed in sandstone from Ohio. The stained glass artist, Wright Goodhue, is known to have created some of the finest stained glass windows in the United States. The 4,721 pipe Skinner Opus 712 organ was built by Ernest M. Skinner and restored in 1997. It is one of four remaining in the United States. A statue is located in the transept of Paul at the moment he recognizes Christ's call and Paul's sword is transformed into a cross. It was created by artists from the National Cathedral in Washington.

The Cathedral inscription over the entrance of St. Paul's: House of Prayer for all Peoples.
