Looking at the 1921 Baist map (below), I'd say that it's the Lanfranco Building.

The 1926 and 1927 CDs still list the Lanfranco Building at 218 N Main Street. The 1926 CD has the San Diego Hotel at 222½ N Main Street, although this has become 222 N Main Street by 1927. Interestingly, the 1927 CD also lists the Bank of Italy (International Branch) at 220 N Main Street.
