The Avala tower (http://skyscraperpage.com/cities/?buildingID=2534) has been rebuilt, according to Wikipedia at http://en.wikipedia.org/wiki/Avala_Tower
How should we handle this in the database?

I propose a new entry. It seems that it has been rebuilt from scratch (photo's wiki link)

I would also suggest a second entry, as it is really difficult to say if a structure was rebuilt in the same manner!

I would handle it just as you did with "Lincoln Cathedral".
