COVID-19 Issues, Actions & Best Practices

This thread is to discuss issues and best practices to tackle infectious diseases especially Coronavirus disease 2019 or COVID-19. Although it focuses on India relevant news from across the world may also be posted.

All candidates for best practices are classified under Best Practices though they are not yet proven to be best. (this line will be removed later).

I am changing 'Best Practices' to 'Actions'. This is because actions are taken against issues without knowing its effect and whether it is 'best action' or 'best practice' is decided at the end only. 'Actions' may be 'Preventive Actions/Measures', 'Mitigation Measures', 'Corrective Measures/Actions', 'Control Actions' etc. Even advisories/advices can be 'actions'. (Decision-making, Issue log , Project Issue Management , Control (management), Control system).

Actions are classified as following
  • 'Control Actions' will be from various government agencies like central government, state governments, local governments at cities & villages, courts and then WHO advices will also be included (will not be subclassified).
  • 'Suggested Actions' are suggestions from non-government experts, opinion makers, self-styled experts/expert-journalists etc.
  • 'Innovative Actions' are disruptive actions taken by public, companies and non-government organizations.
  • 'Volunteer Actions' are from volunteers & voluntary organizations. Because of workload I am not going for further classification but a tracker tool users can do that more easily.
  • 'Private Actions' are actions taken by individuals, companies/organizations/societies/communities for themselves.

It is also interesting to see how 'actions' will result in further chain of issues though I may not be able to link them in this thread. Because of this 'actions' will become 'issues' later. For that matter here I am clubbing so many issues together under a single class of issues.

At the end we can decide about best practices looking at the results 'actions' produce.

To digest virus in stomach by drinking water regurlarly as mentioned in the video above one may have to boost ones digestive power with other ayurvedic/yogic techniques in case suffering from low stomach acid or any other digestive issue. When essential commodities becoming scarce some amount of fasting may be useful to kill germs accumulated in stomach but should be careful not to stress too much.

The technique in the video below is similar to the YPV technique above with supporting reasons.

Supply chain for foods should be strong enough to see that there is no dearth of protein, vitamin C foods which strengthens immune system. People should also be encouraged to expose themselves to morning or evening sun for vitamin D. Traditional medicinal items like amrtaa/amrtaavalli/giloy/guduchi (tinospora cordifolia), tulasi (holy basil, ocimum tenuiflorum), neem, gooseberry/amla (emblica officinalis), turmeric, ginger etc should also be available aboundantly in essential commodities market.

It is best to broadcast 'pratical videos' (showing how to do actual exercises) like above videos on yoga, pranayama, meditation in TVs (including cable TVs) instead of wasting time on discussions only. Though these exercises may not help much in the short run they will be very useful in the long run.

Whether technology can help to overcome the crisis?

If the pandemic dissipates — either on its own or because of these measures — short-term extreme social distancing and lockdowns may be bearable. How long, though, should measures like these be continued if the pandemic churns across the globe unabated? How can policymakers tell if they are doing more good than harm?
Long term planning is required. NITI Ayog has a big task.

1. Supply Chain

Each district should prepare a long term plan for procuring and distributing essential commodities, pharmaceuticals, security gadgets. Huge scope is there for computerized planning - 'ERP for supply chain management' can be deployed at district level by district authorities if not at country or state level. There is scope for developing cheaper versions of biological hazard resilient supply chain. Medical/healthcare logistics, food logistics, fuel/energy logistics, police logistics, humanitarian logistics and all other essential services maintenance act related logistics are priority ones. Location-based delivery services can be incorporated as a part supply chain.

At least all essential commodities industries should be up and running as soon as possible. Many middle class families have already procured groceries for a month. Hence at the end of this there will be another rush for groceries.

District authorities should have details like stock, demand, supply etc on their finger tips. They should also have alternative supply arrangement to meet the demand. All essential commodity industries to meet the demand should be running continuously - Udupi allows operation of rice mills, coconut oil manufacturing units.

E-commerce platform (‘Launch e-commerce platforms to link farmers with consumers’) is integral part of supply chain management. WhatsApp (Now, doorstep delivery through WhatsApp) and other social media platforms also be tried for placing order including cashless (WhatsApp Pay).

Human Resource Management should be a part of districtwise 'supply chain management and logistics' - COVID-19: KSRTC deploys buses for emergency services.

No idea whether district authorities have access to GST Daily Stock Account to check inventory status of the district and whether summary report on essential goods inventory is available.

2. Employment

- IT workers are asked to work from home
- Recruitments go online
- Colleges go online
- Doctors go for telemedicine
- Courts opt video conference
- Online shopping - but Supply issues hit online delivery services
- E-offices - E-offices keep things going as usual in some govt. departments, Laptop sales surge in India as employees work from home during lockdown

But how to employ others like MSME and construction including migrant workers is a big issue. Self-reliant villages generating local employment can be planned including a plan for to and fro movement of goods as a part of supply chain management.

Issue is also of loosing income earner of family - Here's why the coronavirus may be killing more men than women. The US should take note.

We have now completed first phase of lockdown which lasted for three weeks. People have to go through next phase also without anxiety or fear. For this we need to have some more motivational activities to unite nation's souls! Hope like two activities in the first phase PM announces a few more.

One activity I like is doing exercise in sunlight in the evening. This may result in strengthening immunity system by accumulating Vitamin D and also through exercises like breathing exercises. Breathing exercise may also reduce anxiety. Any way any other secular exercise is also fine.

The sixth step, research, has to not only meet these defensive challenges, but also launch an all-out offensive against the enemy. We must innovate at every step, use our fabled “jugaad” as a stop-gap defensive measure, while our premier research institutions work to provide new weapons in this fight.

While the challenges are severe, the unified response of the government, industry and academia in this time of crisis has been heartening. For example, to test widely, we need diagnostic kits. As global supplies of kits and their components start becoming scarce, Indian industry and start-ups, supported by academia, are developing indigenous alternatives. Medical researchers, epidemiologists, molecular biologists, and technocrats are jointly formulating strategies for minimum testing for maximum insight.

Apps like Aarogya Setu leverage India’s digital and telecom infrastructure and tech talent to supplement community testing and provide virtual intelligence. Previous investment in high-quality basic research is also paying off. The recent development of futuristic alternatives such as a Clustered Regularly Interspaced Short Palindromic Repeats (CRISPR)-based paper strip test, which may eliminate the need for expensive and scarce real-time Polymerase Chain Reaction (PCR) machines, came from years of background work.

While issues remain in expeditiously bringing new tech to market — for example, sufficient quantities of the input materials for the CRISPR test are difficult to find — a strong academia-industry combine is working hard to overcome the obstacles. Meanwhile, numerous research labs across the nation are using their infrastructure to enhance the national Reverse Transcription (RT)-PCR testing capacity. Young companies funded by the Council of Scientific and Industrial Research (CSIR) and the Department of Biotechnology industry programmes are coming out with affordable and simple indigenous diagnostic systems that can be used by local centres without advanced infrastructure. Together, these form a viable plan to widen the testing net and effectively determine local and personal risks.

Similarly, procurement challenges in personal protective equipment (PPE), hospital beds, oxygen, ventilators, and newer anti-viral drugs, must be met by focused science. Towards this, Indian research organisations are using cross-sectoral strengths to create the necessary pipeline of products. When biohazard suit production was constrained by the unavailability of seam sealing tape, a sealant glue from submarine applications was substituted by Defence Research and Development Organisation, ensuring that these suits can be produced in large numbers, forming a robust line of defence for frontline health workers. An indigenous oxygen concentrator from the CSIR Venture Centre may make the difference between life and death for hypoxic patients, if central oxygen supply runs short.

Lifesaving drugs, with promise in Covid-19 treatment, are being synthesised by CSIR labs in collaboration with pharma majors. Inspiration from traditional medical systems is guiding the ministry of Ayush’s efforts. The idea is to not blindly copy anyone, but to take the best of what is available and by using universally-recognised scientific principles, modify it to local contexts and needs. We have bought ourselves time to prepare and have the scientific capacity to deliver. This war will be won by smart data-driven defence, coupled with strategic offence.

While the medical side defends with what they have, the research side must support them with what they need. Importantly, we must not take our eyes off the real solution — effective treatment and vaccines; developed in India for the world. We have the leadership to inspire and mobilise the public. Can science guide decisions to shape the public health response and to restart the economy? If we do this well, not only will we save grief to hundreds of thousands of Indians, but also contribute to global good and emerge a stronger nation.

Though I started this thread with issues and best practices I have not covered any best practices so far. I intend to post on lessons learnt, best practices etc soon.

And I have done many errors in classification of actions. I was classifying a few of control actions as issues based on my feelings, but actually I should had classified them as control actions only and then commented on issues. Many of news links though classified under innovative actions they were also either government, private and volunteer actions. Better way would have been combining them as control & innovative actions, private & innovative actions, volunteer & innovative actions etc. Similarly, suggestive actions could have been for government, private companies or volunteers. There could have been actions executed together by government and private companies or volunteers. Anyway doing things perfectly on a blog like this is very difficult task.
